Advanced Practice Provider (APP) - Pulmonary Hypertension & Interstitial Lung Disease
Location: Boston Medical Center and Boston University Chobanian & Avedisian School of Medicine
Are you a dedicated and compassionate Advanced Practice Provider (APP) looking to make a meaningful impact in the lives of patients with advanced lung diseases? We are seeking a skilled Nurse Practitioner (NP) or Physician Assistant (PA) to join our multi-disciplinary team, specializing in Pulmonary Hypertension (PH) and Interstitial Lung Disease (ILD).
About the Role
Patients with PH and ILD require highly specialized care, including frequent outpatient visits, medication management, and close clinical monitoring. Our clinic currently faces high patient volume and extended wait times, making this APP position essential in improving access to care, enhancing patient outcomes, and supporting our expert team of physicians.
Key Responsibilities
- Conduct follow-up visits for PH and ILD patients, improving clinic access and reducing wait times.
- Facilitate urgent care visits and post-hospitalization follow-ups within 1-2 weeks.
- Serve as the primary point of contact for medication management, including lab reviews, medication titration, and physician consultations.
- Assist in patient triage and referral coordination with sub-specialties (Cardiology, Rheumatology, Hematology, etc.).
- Provide medication teaching and administration oversight in collaboration with the PH/ILD RN.
- Coordinate with external facilities, including lung transplant centers and specialized pulmonary programs.
- Schedule and manage procedures such as right heart catheterizations and bronchoscopies.
- Participate in multidisciplinary ILD meetings and document care recommendations.
- Medication teaching and patient education: 1 session/week

Required Skills
Qualifications:
- Licensed Nurse Practitioner (NP) or Physician Assistant (PA) with experience in pulmonary medicine, cardiology, or critical care preferred.
- Strong background in complex medication management and multidisciplinary care coordination.
- Excellent communication skills and a patient-centered approach.
