Provide patients with suggestions and resources on how to improve medication adherence

Contact pharmacies, physicians, and medical groups as needed to help improve overall care of the patient related to medication adherence.

Document call success rates and escalations within the provided excel template
Document successful calls within Practice Fusion EHR
Sign off on ones own documented calls within Practice Fusion

Key Responsibilities

Attend training sessions and call center meetings.
